This position will require the successful candidate to carry out traditional case management duties utilizing a community based team approach with forensic adult SMD clients within Fairfield County. The Case Manager will serve in an integrating role with therapists, a psychiatrist, and the client to ensure that the client's mental health needs are met consistently. This full time position affords flexible hours to ensure that client needs are met. The Case Manager will be expected to spend a minimum of 20 hours a week working directly with clients within the community to address medication, housing, work, and other issues that aid the client in functioning at their maximum level. Requirements: Bachelor's degree in related field.
