Duties:

Responsible for the quality and completeness of the clinical record.

Assesses client needs and formulates a strength-based Individualized Service Plan with the client and their family.

Evaluates and revises the ISP according to client's responses and needs.

Follows crisis prevention/intervention protocols.

Provides support, education and consultation to clients and their families.

Assists clients in gaining access to community resources.

May provide some individual and group counseling as needed.
