POLICY It is the policy of Mid-Ohio Psychological Services to ensure that appropriate authorization for treatment is obtained prior to providing services to clients. Staff must ensure that clients provide informed consent for treatment and this informed consent must be documented. For clients under the age of 18, the parent or legal guardian must provide the consent for treatment. The Clinical Director is responsible for ensuring that this policy is implemented and complied with.
PROCEDURE 1. All clients will be provided a copy of the Client Guidelines prior to initiating services. This document will be mailed to clients prior to their arrival for the first appointment when possible or will be hand delivered to clients upon their arrival for their first appointment. 2. The Client Guidelines will, among other things, explain the risks and benefits of each proposed treatment, will affirm the client’s right to refuse treatment, and will explain the clinician’s role in working collaboratively with the client to ensure that they receive the needed services. 3. Prior to initiating services, the providing clinician is to review the Client Guidelines with the client and ensure the following issues have been addressed: a. explanation of the risks and benefits of each proposed treatment, of alternative treatments, and of no treatment b. agency response to the refusal or withdrawal of consent for treatment including: 1) reaffirmation of the person’s right to refuse treatment according to rule 5122:2-1-02 of the Administrative Code; 2) efforts to develop alternative approaches collaboratively with the person served that ensure the person receives needed services; and 3) efforts to ensure that the person served understands the implication and potential consequences of refusing or withdrawing consent for treatment. 4) documentation of consent for treatment, refusal to consent, or withdrawal of consent shall be kept in the individual client record. c. clients under the age of 18 must have the consent of their legal guardians prior to receiving services 4. Each treating clinician must document that these issues have been reviewed with the client. By signing the Authorization For Services portion of the Client Guidelines, the clinician and the client are affirming that these issues have been addressed. 5. Any time a significant change in service occurs, the treating clinician must review these issues with the client and document that this review occurred in the corresponding case note.
